As of December 2024, UpStream's model had a network of over 1,000 physicians, providing care to over 180,000 senior patients. 

Key customers and partnerships

The company has established partnerships with physician groups across multiple states, including the Community Care Physician Network in North Carolina, Tidewater Medical Group in Virginia, and the Medical University of South Carolina Health Alliance. These collaborations focus on delivering comprehensive, coordinated care to seniors with chronic conditions. 

In January 2023, UpStream Healthcare and Innovacer partnered to more than double its population on Innovaccer's cloud-native platform over the next year, and will use it to scale UpStream's unique value-based care model nationwide.
