Founded in 2020, Cadence offers an at-home remote patient monitoring (RPM) program to manage chronic diseases such as hypertension, heart failure, and Type 2 diabetes. Its proprietary platform “Care in Sync” uses connected devices that track patient vitals, including heart rate, blood pressure, blood glucose, and weight, allowing providers to adjust personalized care plans. The company also connects physicians and patients via call and text for care concerns.
Cadence’s Care in Sync platform provides personalized, data-driven care to patients at home. It operates a B2B2C business model by partnering with healthcare providers, and health plans.
