Founded in September 2022, Harbor Health is a healthcare company that offers both in-person and virtual care services. The company distinguishes itself by integrating primary and specialty health services into a model known as a "pay-vider"—a combination of payer and provider. This innovative model aims to create health plans that actively involve consumers in their care pathways, emphasizing preventive and personalized health care. The company additionally offers its care in a five-step process: 1) making an appointment, 2) being assigned a care team, 3) care navigation, 4) receiving care, and 5) managing health post-consultation. In step two, patients are assigned a doctor and nurse duo that will remain assigned to the patient as a part of their care journey and assist with tasks such as managing referrals, medication management, and care navigation.

In January 2024, the company expanded its operations, growing from two to eight locations in Texas, with more expansion planned for 2024.

Key customers and partnerships

In March 2024, Harbor Health partnered with AvoMD to deploy AvoMD’s AI-powered point-of-care tools into its workflows. Through the partnership, the company will use AvoMD’s Builder platform to incorporate clinical guidelines from AvoMD’s content partners at the American Heart Association (AHA), American Academy of Family Physicians (AAFP), Kidney Disease Improving Global Outcomes (KDIGO), and others into Harbor clinicians’ workflows.

Funding and financials

In January 2024, Harbor raised USD 95.5 million in a funding round led by General Catalyst, along with participation from other venture capital firms. The proceeds were set aside to help support Harbor’s growth of primary and specialty care services. The funds were also earmarked to develop new plan designs that support care journeys.
