ConcertoCare (formerly known as ConcertoHealth) is a primary care provider that operates an omnichannel care delivery model, offering both in-person and virtual care services, including remote patient monitoring to manage chronic conditions. Its proprietary data analytics and clinical decision support platform Patient3D helps determine the best course of action for a patient. The platform consists of three main programs: 1) ConcertoCare Partners, a program designed for the complex needs of patients and primary care providers; 2) In-Home Primary Care Provider, a program designed for users who do not have a primary care provider; and 3) PACE Services, an end-to-end homecare solution. In February 2021, the company merged with fellow home-care provider Perfect Health and was to operate under the name “ConcertoCare.” As of February 2022, Its services were available in eight states across the US.
In February 2022, the company acquired Crown Health, a home-based primary care provider. The acquisition accompanies a Series B fundraise of USD 105 million led by Wells Fargo Strategic Capital.
