ConcertoCare (formerly ConcertoHealth) is a value-based primary care provider that operates an omnichannel care delivery model, offering both in-person and virtual care services to seniors and those enrolled under Medicare Advantage, including remote patient monitoring services to manage chronic conditions. Its proprietary population health data analytics and clinical decision support platform Patient3D offers actionable insights on a patient.
The platform has two main programs: 1) CarePartners In-Home Support, a program designed for the complex needs of patients and primary care providers, and 2) In-Home Primary Care Provider, a program designed for users who do not have a primary care provider.
In February 2021, the company merged with fellow home-care provider Perfect Health. Further, in February 2022, the company acquired home-based primary care provider Crown Health to expedite its growth and improve health outcomes among older adults in the Pacific Northwest region.
As of May 2024, its services were available in five states across the US.
Funding and financials
In February 2022 the company raised USD 105 million via a Series B funding led by Wells Fargo Strategic Capital, which was used to enhance its care delivery model, expand partnerships with value-based health plans, and launch in new markets across the US.
