Praia Health is a digital health company that provides a platform enabling health systems to deliver personalized health solutions at scale. The company's core offering is the Praia Health Consumer Platform, which leverages identity-driven personalization technology to create individualized digital experiences for patients.

Incubated at Providence, one of the nation's largest health systems, Praia Health's platform has been in use within Providence since January 2022. It currently supports over 3.5 million user accounts, offering digital self-serve capabilities, virtual care access, and tailored program, service, and resource recommendations based on the user's health profile.

Praia Health's platform transforms how third-party consumer solutions integrate into a health system's digital infrastructure. Through its low-code ecosystem enablement capabilities, the platform securely manages data exchange between third-party solutions, the health system's electronic medical record system, and other data sources. This approach increases the flexibility and speed of third-party integration while reducing associated costs.

In April 2024, Praia Health secured USD 20 million in an oversubscribed Series A funding round led by Frist Cressey Ventures. Other investors included SignalFire, Epsilon Health Investors, and Providence Ventures. The funding will bolster the platform's ability to drive digital transformation for health systems.

Community Health Network, an Indiana-based health system focused on patient-centric care, has licensed the Praia Health Consumer Platform for implementation in 2024. The platform's novel identity and personalization technology are expected to remove traditional barriers to patient care by seamlessly connecting individuals to the appropriate services, products, and resources.

Praia Health has formed strategic partnerships with several digital health companies to enhance its ecosystem. These include Atlas Health, Cedar, DexCare, Foodsmart, Fortuna Health, Kyruus Health, Livara Health, Medbridge, Modivcare, Omada Health, Rosarium Health, Season Health, Vale Health, Wellthy, Wildflower Health, Validic, and Xealth.

Notably, Xealth serves as Praia Health's clinical integration partner, enabling digital solutions to be incorporated into the clinician experience and extending the ecosystem to over 50 additional connected digital health partners.

Furthermore, Praia Health has secured Panda Health, the AVIA Marketplace, Xealth, and West Monroe as channel partners for its platform.
