Canada-based Bayshore Healthcare focuses on providing care to seniors at their homes. The company provides care with regard to cancer, dementia, diabetes, heart disease, multiple sclerosis, rheumatoid arthritis, and strokes. Bayshore provides care through care planners, in-home nursing, caregiving, therapy and rehab, medication management, and infusion and injection services. The company additionally offers related services, such as a marketplace to procure medical devices and supplies and clinical trial management services, where it helps companies carry out clinical trials with on-site services such as medication administration, specimen collection, data capture, and analysis, along with the storing, dispensing, and destruction of investigational products.

The platform uses the HL7, a standard for health data management, and is paired with Bayshore’s clinical team of dedicated nurses and caregivers, who provide at-home and in-person treatment and interventions. Vantage also includes connectivity via an app and connected medical devices, electronic referrals and treatment monitoring, individualized care plans, and the ability to integrate with existing healthcare systems. 

The company operates across Canada, through 80+ home care offices, 11 pharmacies, and 100+ community care clinics. 

Key customers and partnerships

In May 2023, the company partnered with Microsoft to launch a remote patient monitoring solution. The solution, Vantage, was built utilizing Microsoft Cloud for Healthcare in order to monitor patients, intervene in time, and reduce hospital readmissions and emergency room visits.

In June 2024, the company partnered with SCALE AI, Canada's AI Global Innovation Cluster, to improve patient care through two predictive models; the Patient Segmentation Model, and the Adherence Forecasting Model. The Patient Segmentation Model aimed to categorize patients based on their health conditions and care needs, while the Adherence Forecasting Model would predict patients' medication adherence trends. The solutions assisted in identifying at-risk individuals and allocating resources more effectively, enhancing healthcare delivery efficiency and patient outcomes. Additionally, as a part of the partnership, Bayshore agreed to work with entities such as Deloitte Canada, Architech, and Manifold to develop the models successfully. As a part of the partnership, SCALE AI invested USD 1.9 million in the project, bringing the total funds raised for the project to USD 6.3 million.
