Concert Health is a mental health tech company that offers behavioral health services in partnership with primary care providers through Collaborative Care Management, an evidence-based model for treating anxiety and depression. Primary care doctors screen patients for mental health conditions and refer them to Concert’s care managers to develop a treatment plan with a psychiatrist. 

Concert Health's model connects patients with a behavioral care manager within 24 to 48 hours of referral. These care managers, who are masters-level clinicians, work closely with patients to set goals, develop coping strategies, and provide support for behavioral health medications if prescribed. The company also employs psychiatric consultants who advise the care team on treatment options and review patient cases regularly. Care is delivered primarily through phone or video visits, with some co-located services in larger partner practices.

The company's technology platform includes a proprietary patient registry that helps track symptoms and treatment progress over time. 

Key customers and partnerships

As of September 2024, Concert Health operated in 17 states and had cared for over 70,000 patients, with more than half insured by Medicaid and Medicare. 

In October 2023, the company announced a collaboration with Mass General Brigham to expand behavioral health services across their primary care facilities in Massachusetts, giving over 400 primary care providers access to Concert Health's services. Concert Health also partnered with WellSpan Health in May 2024 to provide same-day behavioral health services to patients in South Central Pennsylvania and Northern Maryland. Other notable partnerships include Mercy Health, where Concert Health expanded its program to encompass 150 clinics and 1,300 primary care providers across Arkansas, Oklahoma, and Missouri in June 2023.

In December 2022, Concert Health partnered with Med First to bring behavioral health services to primary care patients across North Carolina and South Carolina, serving approximately 200,000 patients annually. The company has also collaborated with Benson Hospital in Arizona to provide integrated behavioral health services to rural communities. In October 2020, Concert Health partnered with CommonSpirit Health, a large-scale primary care provider with 137 hospitals across 22 states.
