Tembo Health offers a virtual support team for patients and families living with dementia. This includes an interdisciplinary team of expert providers, nurses, and specialists who are accessible via text, phone, or video and who offer 24/7 concierge support, counseling, education, care coordination, emergency care, and regular monitoring of health and vitals.
Initially, patients (or their families) meet virtually with care navigators to build personalized care plans. The care navigator then works with members to identify and evaluate necessary in-person primary and specialty care providers, assisting with scheduling and helping members understand medical terminology. Through members’ phones, the interdisciplinary team is contactable at all times to coordinate emergency visits or to obtain educational material and tools for better understanding/managing dementia.
