Cityblock Health partners with MDwise to provide primary care to Indiana communities

Partnerships

  • Direct primary care provider Cityblock Health partnered with Indiana-based healthcare company MDwise to provide care services to selected Indiana communities. 

  • Under the partnership, Cityblock will open care clinics for members to provide personalized care for physical health, behavioral health, and social determinants of health. The program will initially launch to support 10,000 Medicaid members. 

  • Non-profit organization MDwise is focused on assisting the vulnerable population in Indiana with healthcare plans using its network of clinicians and health systems. Two health plans provided by the organization are Hoosier Healthwise for children under the age of 19 and Healthy Indiana Plan for adults who are not eligible for Medicaid or Medicare.
