Remote patient monitoring startup Story Health and nonprofit healthcare system Intermountain Health partnered to provide specialty care for patients managing heart failure. Through the partnership, Story Health coaches and Intermountain clinicians will work together to provide patients with heart and vascular-related treatments using Story Health’s EHR-integrated remote monitoring technology.

Through this partnership, patients at Intermountain’s 33 hospitals and 385 clinics will also have access to Story Health’s patient engagement services such as reminders and answering questions, via email, text, and voice call. The partnership is in effect from January 2023, and will also incorporate Story Health’s other heart health services, including heart failure-related home titration, volume management, and advanced therapy evaluations along with its refractory hypertension home optimization services.
