Less than a year after securing USD 53 million, direct primary care provider Vytalize Health raised USD 100 million in a funding round led by Monroe Capital and Enhanced Healthcare Partners. with participation from North Coast Ventures. This brings its total funds raised to USD 170 million.
The funds will be used to hire additional staff, improve technology, and strengthen primary care practices to support value-based care for seniors.
The value-based care platform focused on senior care operates virtual and in-home clinics together, with independent primary care practices in order to manage high-risk populations. Additionally, the platform offers data on patients and practice management tools for clinicians. Currently, the firm manages more than 250,000 senior Medicare patients, partnering with 400 primary care practices across 36 states.
