Aledade and Humana announce 10-year partnership

Partnerships

  • Value-based healthcare provider Aledade announced a 10-year collaboration with health insurer Humana to provide independent primary care physicians within Humana’s network with the necessary tools to provide value-based primary care. 

  • Since 2019, the two companies have collaborated in a few markets and have achieved outcomes that include boosting primary care appointments and lowering inpatient hospitalization by 5% and readmissions by 12%. Currently, over 100,000 patients in Humana’s network are receiving medical care from a physician enabled by Aledade. 

  • In the first four years of their collaboration, the firms aim to achieve the following: 1) increased primary care visits, 2) reduced inpatient hospitalization by 5%, and 3) reduced readmissions by 12%.
