Through the acquisition, PointClickCare will refine its EHR offerings and services, enabling caregivers and care teams to have access to crucial patient information, leading to better patient outcomes.
New York-based Patient Pattern is an EHR and integrated care management platform. The company operates on a B2B basis, working with healthcare providers and specialized patient programs such as Medicare Advantage Special Needs Plans, ACO REACH participants, and PACE programs.
Canada-based PointClickCare offers clinical and administrative workflow management tools to providers while being focused on post-acute and senior care. Its services connect care providers across the US, enabling collaboration and communication, patient tracking, EHR integration, financial management, care transition management, and staff hiring. As of March 2023, the company works with 27,000+ long-term post-acute care providers, 2,800+ hospitals, and 2,000 ambulatory clinics. The company also works with most major health plans and operates across the US.
