Remote patient monitoring provider Reimagine Care has partnered with hospital-at-home provider DispatchHealth to provide virtual-to-home care for oncology patients. Through the partnership, oncology patients will have access to virtual consultations with Reimagine Care's clinicians, in-home care fulfilled by DispatchHealth's care team, and coordinated care between Reimagine Care, DispatchHealth, and the patient's oncology care team.

The partnership was formed with the aim of reducing unnecessary hospital and emergency room visits, and will further support oncology providers through real-time data management, technology platforms, and evidence-based care pathways.
