Chronic care management company Ayble Health has partnered with health system Mayo Clinic to provide a combined virtual and in-person care model for patients with chronic gastrointestinal (GI) conditions. The hybrid care model will be available via employers and health plans.
The new care model combines AI-guided personalization and expert health coaching to address individual patient needs comprehensively. If the patient is matched for in-person care, they will receive initial care at Mayo Clinic, after which patients continue with Ayble for ongoing nutritional and behavioral support in their local community.
