Vytalize Health is a direct primary care provider focused on offering value-based care to senior citizens. Its services are available virtually via an online platform and at home through partnerships with independent primary care practices. The company first launched in 2014 as a primary care practice in New York focused on Medicare members. It later developed a tech-driven solution consisting of a risk-bearing entity, a technology platform, as well as a virtual and in-home clinic. 

The firm began offering that solution to other primary care practices in 2017, which includes data on patients and practice management tools for clinicians. It claims to offer a unique practice-engagement model to help its clients spend more time with high-risk populations. It also acquired the AI-driven, last-mile patient engagement platform MedPilot in 2021.

Key customers and partnerships

It partners with both small and large primary care practices to support their transition to value-based care by providing data about their patient, practice, finance, and enrollment. As of March 2023, the firm managed 250,000 senior Medicare patients, partnering with 400 primary care practices across 36 states.

Funding and Financials

In February 2023 , the firm raised USD 100 million in a funding round led by Monroe Capital and Enhanced Healthcare Partners. The funds were allocated towards  hiring additional staff, improving its technology, and strengthening primary care practices to support value-based care for seniors.
