Intermountain Health, a nonprofit healthcare system, is expanding its partnership with Story Health, a digital health company, to expand virtual cardiology care delivery.

The expansion follows a successful pilot program launched in January 2023 for patients with new heart failure diagnoses. The program achieved 80% medication optimization without clinic visits, reduced readmission rates, and improved patient engagement. In addition, the partnership now includes a hypertension management program launched in April 2024.

This collaboration aims to improve patient outcomes, increase clinical efficiency, and enhance access to cardiology care across Intermountain's health system. The partnership's success was highlighted at the ACC CV Summit in February 2024, where Intermountain's presentation received a Top Ten designation.
