Story Health, a virtual specialty care company, and Guidehealth, an AI-enabled value-based care provider, have partnered to develop a cardiology program for hospital systems and clinically integrated networks (CINs).
The partnership aims to provide coordinated patient care for at-risk individuals with heart conditions like heart failure, hypertension, and arrhythmia. Guidehealth's Healthguides will identify eligible patients and enroll them in Story Health's program, while Story Health's team of cardiologists, nurses, and health coaches will work directly with patients to provide remote care. The program is expected to improve patient engagement, optimize medication management, address socioeconomic barriers, and reduce overall healthcare costs through the proactive management of at-risk patients.
Analyst QuickTake: This marks the second partnership Story Health has entered this year, the previous being in May 2024 with Saint Luke’s Heart Institute, where the providers partnered to launch a supportive program for patients with Atrial Fibrillation (AFib). Story Health seems to be continuing its trend of expanding its market reach via partnerships, a trend that began in early 2023 with the company collaborating with Intermountain Health . During 2023, Story Health also partnered with West Virginia University and Pearl Health .
