Aledade is a value-based healthcare provider that offers population health technology solutions for independent primary care practices to identify and manage their most at-risk patients. It provides the tools, technology, and support to deliver coordinated primary care services under its accountable care organization (ACO) network.

In February 2023, the company acquired Curia, an AI-enabled value-based care analytics platform, to use Curia’s platform to improve its data-gathering capabilities, refine patient-care targeting, and enhance primary care workflow.

Key customers and partnerships

As of May 2024, the company collaborated with more than 1,900 independent primary care practices and over 16,300 clinicians across 45 states and the District of Columbia.

In March 2023, the company announced a 10 year collaboration with health insurer Humana to provide independent primary care physicians within Humana’s network with the necessary tools to provide value-based primary care.

Funding and financials

In June 2023, the firm completed a USD 260 million Series F funding round led by Lightspeed Venture Partners, which was allocated towards the growth of Aledade's primary care network, strengthening strategic alliances with health plans, and enhancing technology and support for partner practices and patients.
