Remote patient monitoring startup Story Health operates an AI-driven virtual cardiology platform and connected devices targeting patients suffering from heart failure. The company’s offerings include medication management, recommendations for heart transplants and other implants, identification of volume overload risks, as well as recommendations based on patient vitals. The platform uses both a web portal as well as an SMS interface, allowing clinicians and patients to carry out virtual consultations.
The company operates on a B2B2C model, where it also enables clinics to store and send patient data, carry out consultations, and monitor patients. The company also provides some services directly to patients such as referring health coaches, who in turn, provide logistical assistance and act as a direct link to clinicians.
Key customers and partnerships
In August 2024, Story Health expanded its partnership with Intermountain Health to expand virtual cardiology care delivery. The expansion followed a successful pilot program, which achieved 80% medication optimization without clinic visits, reduced readmission rates, and improved patient engagement. The partnership expanded to incorporate a hypertension management program launched in April 2024.
In July 2024, Story Health partnered with Guidehealth an AI-enabled value-based care provider, to develop a cardiology program for hospital systems and clinically integrated networks (CINs). The partnership aimed to provide coordinated patient care for at-risk individuals with heart conditions like heart failure, hypertension, and arrhythmia. Guidehealth's Healthguides would identify eligible patients and enroll them in Story Health's program, while Story Health's team of cardiologists, nurses, and health coaches will work directly with patients to provide remote care.
Also in July 2024, Story Health partnered with Zip Health, an insurance company for Medicare Advantage beneficiaries, to offer a specialized care program for at-risk members with cardiac conditions. The collaboration aimed to identify eligible Zing Health members at high risk of poor heart failure outcomes and connect them with Story Health's program.
In May 2024, Story Health partnered with Saint Luke's Mid America Heart Institute to launch a supportive program for patients with atrial fibrillation (AFib). The new program aimed to address the gaps in the treatment and diagnosis of AFib, a common form of cardiac arrhythmia. With AI and virtual care, the program enables patients to stay connected with the clinical team even outside clinic visits, supporting their care regimen. The program includes AFib-specific treatment plans, lifestyle support, risk factor modification, and medication monitoring.
In September 2023, the company partnered with the West Virginia University (WVU) Heart and Vascular Institute to improve access to specialty care for heart failure patients. Through the partnership, Story Health's platform was integrated into the WVU Medicine system, allowing clinicians to provide care to their patients via Story Health’s platform. The platform integration provided remote monitoring, medication organization tools, and a connection to Story Health’s care team via SMS across. This integration was made available in all of WVU’s 32 care clinics, which span across five states and provided care for around 375,000 patients in 2022.
